Welcome to Tampa Bay Hearing and Balance Center. In order to register for an appointment please call us at 813-844-4900. You may also use the Contact Us page to communicate with us. The New Patient Packet contains important information including insurance information, questionnaires, and directions to our office. Please download this packet and have it filled out prior to your arrival.

General Instructions
Click the above link to open the registration forms in Adobe PDF format. This form can be filled out on your computer screen. Make sure you have "highlight fields" turned on. Fields will be in a light blue color. Once complete click on the save icon and save the form to your desktop. You may then email or fax the saved form to us per the instructions within the registration packet.

This image shows the save routine that occurs when you click on the upper left hand side save icon. Give the file a meaningful name such as your name so that it is easy to find the file for emailing or faxing.
