I find the "soft" cochleostomy which does not enter the cochlea with a drill to be quite difficult. This technique attempts to create a blue line from anterior to inferior. That leaves a bony island which can be cracked out. The concept I tried to copy was Howard House's double blue line which allowed him to crack off the lateral prominence of the LSC for fenestrations.
